Members Only Contact Sheet: This pdf has all the digits to call for Member Services, Benefit Explanations , Pharmacy, Vision, Traveling Outside of California, Dental Information etc. Includes key mailing addresses too.

Tonik Members VIP Lounge: Takes you to a new page to order a new card, manage your account etc. You will have to log in using the username and password you created when you first applied.

Dr. Finder: Takes you to new page to find doctors, specialist and hospitals in the network. Remember that Tonik is a Anthem Blue Cross of California Prudent Buyer PPO

Automatic Credit Card Payment Form: Use this form if you want to set up monthly payments from your CC.

Automatic Direct Deposit Payment Form: Same as above but uses your checking.

Medical Claim Form: For medical claims

Dental Claim Form: For dental claims

Cancellation Form: Don't leave us!!!! If you are looking for a less expensive alternative, give us a call at 888 285-6334. If you still want to cancel , you will have to fill out this form. The good part is that there is no long term contract or early termination fees with Tonik.
